In order to file your claim, you MUST print either the online prepared claim form or a blank claim form, complete all open sections of the form, sign the claim form, and then mail it with all supporting documentation to the address listed on the claim form. In both cases, a Proof of Claim shall be deemed to have been submitted when actually received by the Claims Administrator, rather than when prepared.

Acceptance of online claim form submission ended August 31st, 2007. You may still download, complete and submit a claim form for consideration.
